I hid behind a tree in the hospital courtyard, watching Derek sit on a bench with a girl in pink scrubs. She was biting into a strawberry cupcake he'd handed her.
Derek smiled as he wiped cream from the corner of her mouth with a napkin, the gesture intimate and tender.
My fingers clawed into the bark until splinters jammed under my nails.
"So that's what all those 'overtime shifts' were about... feeding cupcakes to some other girl."
I stormed out from behind the tree, my voice shrill with rage.
"Derek! How could you do this to me? This is your so-called emergency surgery?"
Derek froze in shock.
"Beth? What are you doing here..."
But the nurse suddenly jumped up, her face lighting up with excitement.
"Beth?"
The air went dead silent.
I looked at her name tag: "Katie Mitchell."
Katie pulled down her mask, revealing the same dimples as Derek.
"Bro, you didn't tell me Beth was so fierce!"
Derek rubbed his forehead and pointed at Katie.
"My little sister. Just started her internship last week."
I laughed awkwardly and shook her hand, feeling like a complete idiot.
Derek flicked Katie's shoulder, making her wince.
"Watch your mouth, kiddo. And stop changing my phone password like yesterday, or I'll let Beth kick your ass!"
Oh. That explained everything.
I'd been so wrong about him. After everything he'd done for me, I'd doubted him. I wanted to crawl into a hole and disappear.
At the restaurant next to the hospital, I treated Katie to lunch.
Katie held up her phone and scooted closer to me.
"Beth, I want to document my internship experience. You're good with photos, right? Make me look cute! My brother makes me look like a zombie!"
I smiled and nodded. I was actually pretty decent with a camera.
Katie posed in various positions by the window, with the hospital entrance and courtyard in the background.
The moment I pressed the shutter, a man in a baseball cap suddenly stopped outside, staring straight into the lens.
I fumbled and nearly dropped the camera. When I looked up, he was gone.
Seeing my panic, Derek came over to check the photo.
In the image, behind Katie, the window reflection showed a blurry figure—a ratty baseball cap shadowing half of a pale, gaunt face.
The homeless freak's face.
I jumped up so fast I knocked over my miso soup, trembling all over.
"It's him... he's not dead."
Derek immediately rushed outside, but the man had vanished.
On the outdoor table where the stalker had been standing, there was an envelope with a message scrawled across it.
